Establishing a relationship with a Primary Care Provider (PCP) is one of the most important steps you can take toward better health. A PCP will ensure you receive timely and coordinated care when you need it. If you’d like to select a provider in the Asante Health Network, we’re here to help. Lower Your Out-of-Pocket Costs
When you receive services from an Asante Health Network (AHN) provider or facility, your out-of-pocket costs like deductibles and copays will generally be lower, when compared to seeing a non-AHN provider or facility.

Get Same Business-Day Access
See your Asante Health Network primary care provider right away when you’re not feeling well or need to treat a minor injury. Once you’ve established care with an AHN provider, you have same-business-day access to their office — simply call and let them know you’re an AHNPlus member. For calls after 1pm, appointments may be offered for the next business day. Should your provider be unable to schedule you, please call the AHNPlus Access Team at (541) 789-1234.
Get Connected Through Asante MyChart
Asante’s MyChart patient portal is a one-stop online avenue to communicate with your doctor, access test results, manage your appointments or make payments online. Many providers within the Asante Health Network are connected through MyChart, streamlining your communication and care.

Receive Health Coaching
You may be eligible for health coaching services through your AHNPlus plan. Complete a Healthy Lifestyle Assessment, and choose a coaching program to support your individual health goals. Coaches may also coordinate with your primary care provider and facilitate referrals, lending additional guidance on your journey to better health. Get in touch with your employer’s Human Resources team to find out if you qualify.
Schedule Video Visits
Experience the convenience of telehealth. Many Asante Health Network providers now offer video visits. To schedule an appointment, please contact your provider’s office. Or, if you have a MyChart account, you can schedule video visits with your AHN primary care provider online.

Frequently Asked Questions
How do I enroll in AHNPlus?
AHNPlus is solely offered through employer health programs in Jackson and Josephine counties at this time. If your company is enrolled, you will be able to select an AHNPlus health plan as a part of your employer’s annual benefit enrollment process. Check with your employer’s Human Resources department for information on open enrollment.
How do I know if my provider is in the AHNPlus network?
You can search our network of 750+ providers by Group or Specialty on our Find A Provider page. If you do not know your provider’s group, you can also visit your employer’s third party administrator website (get in touch with your employer directly for the link), or contact your current provider directly.
Do I need to choose a primary care provider?
Selecting a primary care provider (PCP) is not required under AHNPlus. However, there are many benefits to having a PCP. This provider is the expert in working with you to manage your overall health, and when needed, they will coordinate your care with specialists.
AHNPlus makes it easy to establish care with a PCP. Please call the New Patient Access line at (541) 789-1234, and we’ll help you select a provider and get you scheduled with your first appointment.
How do I get care from a specialist?
AHNPlus gives you the freedom to directly schedule appointments with specialists, subject to the requirements of each provider. Authorizations for specialist visits are determined by your employer’s health plan. For these reasons, we recommend going through your AHN primary care provider for specialist appointments, as they can take care of the necessary paperwork and follow-up.
